Incorporating a Perioperative Surgical Home (PSH) into your practice can be challenging, but there has never been a better time than now to receive expert support from thought-leaders within this burgeoning model of care.
As medicine continues its shift from volume to value, the PSH is increasingly being adopted by organizations seeking a platform to help facilitate a patient-centered, physician-led, interdisciplinary and team-based system of coordinated care.
The all-new Perioperative Surgical Home Consult program offers a customized approach to your own care redesign. Whether you are at the very earliest stages of a PSH or have a PSH model under way, the Consult program will help you to:
- Identify and educate key leaders and stakeholders within your organization
- Assess your organization’s capabilities and pinpoint opportunities for further development
- Develop a prioritized action plan with supporting resources
There are four components to the PSH Consult: 1) Pre-assessment Webinar, 2) Assessment Tool, 3) Post-assessment Webinar and 4) a CME Evaluation required to secure CME credit.
A PSH Consult took kit can be found on the PSH website, including FAQs, an informational overview presentation PowerPoint and flyer, and a C-suite presentation/talking points document.
The assessment process consists of nearly 100 items and can be completed using a variety of approaches, depending on your needs: 1) Champion-led Approach, 2) Individual/Then Group Approach, 3) Group-Only Approach and 4) a Divide-and-Conquer Approach. Completion of the assessment can take hours or up to several weeks, depending on the number and availability of your assessment participants.
Collaboration and progress will be maintained on a secure website, where participants can communicate, review content, monitor progress and capture CME.
