Health care institutions of all types and sizes are charting a new course for the Perioperative Surgical Home (PSH) model and sharing their journeys for the benefit of others. Multiple, diverse case studies will comprise a new six-part online education series, “Cases From the Perioperative Surgical Home: A Journey to Improve Quality and Patient Safety.” Rob Shakar, M.D., Medical Director, Perioperative Surgical Home at New Hanover Regional Medical Center, will present the first case at Sunday’s session.

The PSH is a patient-centric, team-based model of care created by ASA to help meet the demands of a rapidly approaching health care paradigm that will emphasize gratified providers, improved population health, reduced care costs and satisfied patients.

Rob Shakar, M.D., will share his experience with the PSH model.

These cases will focus on the institutions’ journey to implement a PSH pilot and its impact on the improvement on patient care and outcomes. Participants will learn more about the barriers and challenges facing health care institutions in implementing a PSH pilot, as well as solutions for overcoming those challenges.

Among the issues discussed during the session will be a look at current and upcoming policy changes influencing the shift toward PSH models. Individual case studies will pinpoint components of effective teamwork, population health management and organizational change strategies and tools.

ASA has partnered with Premier Inc., a health care improvement company, to develop the PSH Learning Collaborative.  ASA and Premier recently announced the launch of the collaborative’s next phase — PSH Learning Collaborative 2020.

PSH Learning Collaborative 2020 will operate over a two-year period. The program includes two participation options:

• The Core Collaborative, designed for those who have recently launched or are in the early stages of launching a PSH pilot
• The Advanced Cohort, which is designed for those institutions looking to optimize and/or expand their PSH pilot.

Since ASA and Premier launched the PSH Learning Collaborative, health care institutions have achieved measurable improvements in the cost and quality of surgical procedures through reduced complications and readmission rates, as well as streamlined operating processes that maximize savings.

A number of participants in the collaborative have already shared their stories of success.

For example, a regional medical center was able to reduce readmission rates by 43 percent, while a Midwest academic center reduced length of stay for admissions related to total joints from 4.6 to 2.1 days. Another participant reported a realized savings of $1.5 million in the first year of joining the program, focusing on performance improvement in key areas identified by the PSH program. A major health system reduced its length of stay for hip and knee arthroplasty from an average of 110 hours to 51 hours.
